If you need a copy of your medical records or test results from our office, you may request them at any time. You will need to complete the release form, making sure that you provide us with how you want to receive your records. Once we have received your completed release form, it will take five to ten business days to process your request. If you requested to pick up your records, you will be notified when they are ready. Please be aware that you will be asked to show your ID when picking up your records. We can also mail or fax your records to the address or fax number provided on the release form.
If you had tests done in another medical facility or physician's office that pertain to your appointment with us, make sure that you bring the results with you. Any outside medical information that you bring with you will be valuable information for our physicians. It will decrease the amount of testing required and help better diagnosis your problem.
If you are unable to get your medical records or test results prior to your appointment, you might want to consider rescheduling your appointment until you can have them available.
