Our staff will be more than willing to assist you in completing your forms. However, you should complete your part prior to submitting them to us for completion. We do charge a $20 fee to complete the forms for you in order to defray some of the cost for administrative services.
Patients are asked to submit their forms during our normal office hours from 8:30 am to 5:00 pm. You may submit them at either our main office on Hamill Road or at our satellite office on Gunbarrel Road. You may also bring them at the time of a scheduled office visit. All FMLA papers will be completed by the physician's nurse after any scheduled surgery has been completed so that they will be accurate.
Please make sure that you give us instructions for returning them to you by mail or if you would like to pick them up, make sure you give us a current phone number so that you can be notified when the papers are completed.
