Medical Records and Testing

If you need a copy of your medical records or test results that you have done in our office, you may request them at any time.  You will need to complete the release form, making sure you provide us with how you want to receive your records.  Once we have received your completed release form, it will take 5 to 10 business days to process your request. If you requested to pick up your records, you will be notified when they are ready.  Please be aware that you will be asked to show your ID when picking up your records.  We can also mail or fax your records to the address or fax number provided on the release form. 

If you have had tests done in another medical facility or physician's office that pertains to your appointment with us, make sure that you bring the results with you. Any outside medical information that you can bring with you will be valuable information to our physicians to help decrease the amount of testing that they may order and also help better diagnosis your problem.

If you are unable to get your medical records or test results prior to your appointment, you might want to consider re-scheduling your appointment until you can have them available.
